If you are insured under an Assurant Employee Benefits Long- or Short-Term Disability policy and you become disabled, a claim should be submitted using these forms. Both the LTD and STD claim statement forms have detailed instructions to assist you by placing the cursor over the field label.
There are some situations where additional information might be required, such as medical and/or pharmacy records, details regarding your job duties and payroll records. If additional information is necessary, a Claims Examiner will notify you upon receipt and review of your claim.
