Voluntary LifeAssurant Employee Benefits offers a $10,000 unit plan or a multiple of earnings plan.

With the unit plan, a covered person can elect a minimum or $20,000 to a maximum of $500,000 in the appropriate unit chosen by the employer (not to exceed five times his or her salary), with or without matching AD&D.  With a multiple of earnings plan, a covered person can elect one to five times his or her salary to a maximum of $500,000 with or without matching AD&D.

Minimum participation is the greater of 10 lives or 20 percent of the group.  Guaranteed Issue (GI) based on eligible employees varies by plan.  Spouse GI is the lesser of 50% of the employee's GI amount or $50,000. All child coverage is Guaranteed Issue.
