Attention Valued Provider: Have you applied for your National Provider Identification (NPI) number? If you submit claims electronically, you are required to obtain an NPI number. You can apply for your NPI at https://nppes.cms.hhs.gov/NPPES/Welcome.do. NPI Enumerator can also be reached by telephone at 800.465.3203, email at customerservice@npienumerator.com, or by mail at NPI Enumerator, PO Box 6059, Fargo, ND 58108-6059.
Effective May 23, 2007, all providers are required to provide their NPI when filing an electronic claim. To expedite the processing of your electronic claim, make sure to include both your NPI number and taxpayor identification number (TIN) on your claim form. The TIN number is needed for tax reporting purposes. Note: In Minnesota, providers are required to include the NPI on both electronic and paper claims.
