Designating a new Primary Family Dentist

You should select a Plan Dentist for yourself and every eligible member of your family when you initially enroll in the Assurant Employee Benefits Dental HMO Plan. You may select a general Dentist using the Find a Dentist online tool. Your Plan Dentist must be a general Dentist, not a Specialty Dentist. Also, you may call Member Customer Service at 800.443.2995 (hours of operation 7:00 am - 5:00 pm CT, Monday - Friday) to select your Plan Dentist.

If Assurant Employee Benefits receives your selection by the 10th of the month, you will appear on the selected Dentist’s roster by the 1st day of the next month. If your selection is received after the 10th, you will appear on the roster the 1st day of the second following month.

You may visit your designated Plan Dentist as soon as you appear on their patient listing/ roster. Please confirm at the time of making your appointment with your Plan Dentist that you are on their roster.

How to change your Primary Family Dentist

To change your Plan Dentist, go to Find a Dentist to locate a general Dentist near you and then contact Customer Service at 800.443.2995.

Find a Plan Specialty Dentist

You may seek treatment from a Plan Specialty Dentist without a referral from your Plan Dentist. Go to Find a Dentist to locate a Specialty Dentist and then call the Specialty Dentist's office to make an appointment. The Plan includes specialty benefits; go to the Specialty Benefit section for more information.
