Filing a claim for Specialty Care
You will need to file a claim form if you use an out-of-network specialty dentist for procedures listed in the Specialty Benefit or for emergencies. You must pay for services at the time the services are rendered and then submit the completed claim form to Assurant Employee Benefits.
For emergency claims, please note on the form “Emergency Reimbursement”.
