Prepaid Plan

Who is eligible?
You, your spouse, and all unmarried children to age 19 are eligible. Also eligible are those unmarried children from age 19 to the end of the calendar year in which they turn 25, if they (1) depend on the employee for support, and (2) live with the employee OR are classified as a full-time or part-time student.
What if I need to change dentists?
With the Prepaid Plan, you may change dentists with a phone call to the Customer Service Department at 800.443.2995.
What are Copayments?
Copayments are reduced fees that are paid directly to the dentist for selected dental treatments.
Is cosmetic dentistry covered?
We understand the importance of a healthy, attractive smile. That's why we have included many benefits for cosmetic services, such as bleaching and bonding procedures.

Insured Plan

Do I have to use a certain dentist?
No. Under the Insured Plan, you may use any dentist you choose.
What is the Allowable Charge?
Allowable charges are based on charges being made by providers in the area where the dental service is performed.
Will my dentist file my claim?
Your dentist can file the claim for you - in fact, most prefer to. We can reimburse you, or the dentist, whichever you prefer.
If my spouse is covered through his/her employer can I cover him/her on my plan?
Yes. The insured plan will coordinate with other group insurance plans to reimburse your out-of-pocket expenses.
